I upgraded to a factory tilt. It definitely helps. I didn't like the feeling of the wheel being in my lap, and it wasn't as comfortable getting in and out. When tilted all the way up, it also gives a little more space front to back as well as room to stretch out my arm more when driving. Makes it feel more like a modern truck that has much more space.
You may also check the foam in your seat. If you had it redone, or somebody added who knows what under the cover to get by, you could easily be sitting much further up and forward than you should be.

My 83 C-20 has a factory tilt column.
I love it.
Now, I am looking for a factory tilt from an 86 with 3 speed auto trans and the wiper switch on the stick to put in my 84 K-10.
