1989 350, 194,000 miles origional injectors, spray pattern looks good, runs good, gas millage not great! Can or should I clean the TBI in a Ultrasonic Cleaner? and replace gaskets?

I'd run your favorite fuel system cleaner through the gas tank and not worry too much about it.

The gas mileage wasn't great to begin with.

My 1990 4x4 burb 350tbi is no spring chicken with 235k miles, but I keep things well tuned up and maintained. I get 12.5-13.5mpg generally cruising at 73-74mph (as long as it isn't really windy) with tires that are about a measured 32in with 3.73 gears. City driving is 10-11mpg.

If you want much better than that, drive 55mph like the speed limit was when the truck was made

Define gas mileage not being great. If the fuel spray pattern is nice and the engine runs smooth with no smoking or bucking/sputtering, I wouldn't touch it. Maybe run some injector cleaner in the next tank, I run B12 Chemtool through all my vehicles a couple times a year

If it ain’t broke, don’t fix it.

I don’t have much confidence in the injector cleaner in a can. Anyone who has tried to remove carbon deposits from valves with a wire wheel knows how difficult that can be. If a wire wheel on a grinder takes a while to remove carbon, what good will cleaner in a can do?
