It's a little easier on a 4x4 due to accessibility to the bolts. 2WD are difficult enough with no engine & even more complicated with an engine, tranny, exhaust, starter, shifter & what else may be in the way.
I have done both either with or without the engine. The last was on my 76 K10 with full accessories. I did not need to remove exhaust.

No need to remove exhaust. I remove the distributor cap, place a hydraulic jack on the front axle below the oil pan. Make sure to put a 3/8" flat piece of steel on top of the jack between it and the oil pan. Slowly jack up the engine until you can remove the through bolts. Unbolt the three bolts holding your small block motor mounts to the frame. Take out the old. Install the new. One trick that I use is to "hang" the new mount on the engine bracket with the through bolt. Leave the nut loose on the through bolt. Slowly lower the jack, dropping in the three frame/mount bolts on each side as you bring the engine back down. I have always found it easier to do this. Than to try and get the through bolt back in on each side. Lots easier to move engine this way. Hope that helps.

I've never thought about installing the through bolts first before. I can see where that could definitely help when installing some of the "less than perfect" mounts from the parts stores.
