Looking for Vacuum Tree/Tee/Tap

Can't find it anywhere. I'm not even sure if it's stock. When I got the truck it was on there. I took a crescent wrench to it (someone used Locktight) when I was doing some maintenance.
I was thinking of just using a T fitting for the hose to the stuff I need hooked up and JB Welding the hole shut.

Plus, it's hard to see but it's slightly bent. I SWEAR IT WAS LIKE THAT I SWEAR

The big nipple goes to the HVAC Ball for my blend doors.
The top one goes to my Transmission Vacuum Line.
The Top Left one goes to my Tip-In switch for my ESC (Knock avoidance).
The Bottom left one goes to my Transmission Switch (For the Torque Converter Lockup).

I'm fine JB Welding this because I can just hook up my HVAC, Tip-In, and Transmission switch. I deleted my cruise control because It just made it look ugly under the hood and I'm never going to use it (plus it had a hole in it, so I doubt it even worked). I'm just more worried about it somehow leaking from the bend.
