Why did some models come with the narrow or wide variety? I have both available but not sure what I should mount on my 83 k20.

I thought they were all the same width regardless of year.
I did too until I found a narrow one in the bone yard and saw pictures with different size. I think @PrairieDrifter is correct. I’m a little partial to the wider version.

For the two tone paint, white on top, where do the two different colors line out in the door jams?

Assuming you’re keeping the drip rails, color change under the drip rail IMO. And top of windshield on the A pillars.
No drip rails, I’d stop the white just as it rounds the corner from cab to door jamb. So door jambs look body color when you open the doors but the roof looks all white with the doors closed.
I’m sure someone will post an actual pic pretty quick for you though.

Interesting, I’ve never seen a narrow trim band under the back window.
I added one to the blue truck (one of the few factory options it didn’t have…)
But mostly to hide dents and oil canning that I didn’t want to spend days fixing and/or pile too much bondo in.

@mxer147 I just went and snapped this pic of the band on my 1987. This one is absolutely original to the truck and should represent the version used on the slant nosed trucks.
