Need to fix or check base items before proceeding. Carb cleaner into carb should have some effect, raise RPM if lean, lower if rich. PCV works on vacuum. Know you said vacuum leaks were checked but try disconnecting anything that uses vacuum except advance assume you checked that, brake booster all of it. Check fuel pressure and volume. With a "mild" cam vacuum might be lower. If you're from Mississippi doubt altitude has anything to do with it. Motor should be in the 16-18 range possibly higher. Think about it this way. How many new trucks would have been returned to the dealer if this was normal? GM did not install electric pumps; they didn't need it. Odd you say idles fine in N and bad in drive? That's usually only a 2-300 RPM change. Check EGR. Most times a finger can be used to pull up diaphragm.
